Brock Huard’s Virtual Football Camp

Brock Huard is bringing his yearly football camp to YOUR backyard with “Brock Huard’s Virtual Football Camp” brought to you by 710 ESPN Seattle. It’s all happening the week of July 27^th. Parents, it’s your chance to sign your kids up for a week-long virtual camp with Brock and former Seahawks Michael Bumpus, Paul Moyer and Dave Wyman leading your youth through drills, instruction, and a little summer encouragement through a series of training videos.

Please register one child at a time below. By completing this form, you are registering for camp and can expect to receive email communication from 710 ESPN Seatte re: Camp. Youth ages 8-18 years old only, please. Prior to camp you’ll receive an email with a private link to our training videos on YouTube. Questions? Email Sea_Promo@bonneville.com.

INFORMED CONSENT AND LIABILITY WAIVER RELEASE FOR
PARTICIPATION IN THE 710 ESPN SEATTLE YOUTH FOOTBALL CAMP

I agree and consent to the following:

I am the parent or legal guardian of (child’s name listed above) who will be voluntarily participating in the 710 ESPN Seattle promotion entitled, “Brock Huard’s Virtual Football Camp” (also known as “Youth Football Camp” or “Promotion”). I recognize that the Promotion requires physical exertion that may be strenuous at times and may cause physical injury and I am fully aware of the risks and hazards involved.

I understand that it is my responsibility to consult with my minor children’s physician prior to and regarding their participation in the above-mentioned Promotion. I represent and warrant that my minor children have no medical condition that would prevent them from participating in the Promotion.

I agree to assume full responsibility for any risks, injuries or damage known or unknown which my children may incur as a result of participating in the Promotion. Such injuries may include, but are not limited to, heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, shin splints, heat prostration, injuries to knees, injuries to back, injuries to foot, or any other illness or soreness, including death.

I knowingly, voluntarily, and expressly waive any claim I may have against Bonneville International Corporation for injury or damages that my minor children may sustain as a result of participating in the Promotion.

I, my heirs or representatives forever release waive, discharge and covenant not to sue Bonneville International Corporation for any injury or death caused by their negligence or other acts.

I have read the above waiver and release of liability and fully understand it contents. I voluntarily agree to the terms and conditions stated above.
